Description:
The RN implements professional nursing practice centered on the patient as a whole person.

The RN coordinates, ensures, and provides direct patient care activities through:
- Provision of nursing rounds, assessments, and treatments.
- Facilitating admission, transfer, and discharge activities.
- Provision of patient and family-centered education.
- Care coordination and individualized treatment planning as applicable.
Job Responsibilities:
Assessment of patient needs is conducted.
A comprehensive assessment is performed and documented in accordance with RN's scope of practice and hospital policies/procedures.
The patient's culture, age, and developmental level are considered.
Patient risk factors and presenting complaints are identified.
Patient health information is obtained from family or others as necessary.
Objective/quantitative and subjective/qualitative data is collected.
The initial admission assessment, if obtained by other members of the healthcare team, is verified.
A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate.
Actions and interventions relevant to the patient's culture and age are designed.
Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care.
Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy within scope of practice.
Procedures, treatments, and processes are explained to patients and families.
Actions to alleviate patient stress and discomfort are initiated.
Changes in patient condition are recognized, responded to, and communicated appropriately.
Ongoing assessment and response to procedures and therapies are conducted and documented.
Tasks are delegated appropriately and oversight of team members in the performance of activities is provided.
Accuracy and completeness of delegated patient care provided by team members are monitored.
Educational needs of patients related to prioritized problems are identified and addressed according to their level of comprehension.
The patient discharge process is facilitated by executing discharge orders in a timely manner.
Potential discharges are identified, and discharge teaching is performed.
Coordination of the plan with Case Management and others is managed as indicated.
Input into patient/nurse assignments is provided in consideration of staffing, unit activity, and patient acuity.
Active participation in unit-based activities in support of organizational values/priorities is demonstrated.
A positive group dynamic and healing environment are promoted through words and actions.
Nursing excellence is assured in accordance with accepted standards, evidence-based practice, current research, and a caring philosophy.
