This role incorporates the principles of Relationship Centered Care and the nursing process. The nurse assesses and evaluates patient care needs and applies critical thinking skills. Integration of relevant assessment and intervention skills is essential in delivering nursing care. Collaboration with the interdisciplinary healthcare team is necessary for developing, implementing, and evaluating the care plan. Prioritizing patient care, including teaching, planning for post-hospital care, and delegating tasks appropriately, is crucial. Patient and caregiver education must consider their readiness to learn and promote health. Effective communication with patients, families, and healthcare team members is essential. Assignments should be managed within the Care Delivery Model to allocate resources effectively. Role modeling of Relationship Centered Care practices is required. Understanding and respecting patients' values, preferences, and cultural diversity must be prioritized. Establishing and maintaining therapeutic relationships with patients and families is imperative. Service recovery for patients and families with concerns must be initiated and escalated as needed.
