Job Requirements

• Active Registered Nurse license
• Minimum Two (2) Years Bedside Clinical Experience highly desired
• ICU experience required
• Critical care experience required
• BLS certification required before first shift
• ACLS certification required before first shift
• Strong assessment, documentation, and communication skills
• Ability to work independently while contributing to a collaborative healthcare team

Responsibilities

• Perform comprehensive nursing assessments and use findings to develop individualized plans of care
• Monitor, document, and communicate patient progress accurately and promptly
• Provide safe, compassionate care to medically complex patients in an LTAC setting
• Collaborate with physicians, charge nurses, and the interdisciplinary care team to support continuity of care
• Support rehabilitation and recovery goals through patient-centered nursing interventions
• Educate patients and families on treatment plans, care needs, and discharge preparation
• Supervise and direct ancillary nursing personnel as appropriate
• Maintain a safe care environment by following infection control and patient safety standards
• Assist with discharge planning from admission through transition of care
• Contribute to a positive, efficient unit culture focused on teamwork and quality outcomes
