Job Requirements

• Active Registered Nurse license or eligibility to practice as an RN in Virginia
• Minimum of 2 years of bedside clinical experience
• BLS certification required prior to first shift
• ACLS certification required prior to first shift
• Strong assessment, documentation, and patient education skills
• Ability to work independently while contributing to a collaborative interdisciplinary care team
• Experience supporting rehabilitative, med-surg, or long-term acute care patients preferred

Responsibilities

• Deliver safe, compassionate, and high-quality nursing care to assigned LTAC patients
• Complete timely nursing assessments and develop individualized plans of care
• Monitor patient progress and communicate changes in condition to the care team
• Document assessments, interventions, and patient responses accurately and promptly
• Support discharge planning, patient teaching, and continuity of care from admission through discharge
• Collaborate with physicians, charge nurses, therapists, and interdisciplinary team members
• Supervise and delegate appropriately to LPNs, CNAs, and other support staff
• Promote a safe, organized, and positive care environment for patients and staff
• Contribute to a patient-centered rehabilitation approach focused on quality outcomes
