First Connect Health is seeking a travel nurse RN Long Term Care for a travel nursing job in Berlin, New Hampshire.

Need RN for Nights 7p-7a 36 hour GTE OT Available Initial 13 week contract - extension possible with good performance - Licensed for 80 Beds, now 74 when full - Census of Skilled: 10-12 - Memory Care: 27 beds, secured unit - Remainder is LTC: 1st Floor 17 2nd Floor 30 3rd Floor 27 - Charting: My unity- Net Smart - Days LPN Ratio: 3 nurses and 2 med aides - Evenings: 3 nurses and 2 med aides - Nights: 2 Nurses (1:20), 2 med aides. Moving FW with 1:64 (average census as of 2/16/23) on nights only for nurses with 2 med techs/aides - Shift times: 7a-7p, 7p-7a. All stuff must be flexible/Variable if needing to float for an opposite shift - 7 minutes past the hour you are considered late. Call Outs: Do not call On Call phone after hours. Will need to call main line if working eve/nights and the need to call out. The On Call/emergency phone is not manned during during business hours. At least 4hrs in advanced, helpful if you can find own replacement. Cell phone policy: When travelers are oriented they sign their expectation sheet (saved under facility files). It clearly states that cellphone will not be allowed in resident areas. They have had complaints that ?travelers? are using phone and earphones to talk with family and friends during shifts and resident care. This will not be tolerated and they will be disciplined accordingly. Please remind travelers of our expectations at our facility. Schedule app: "When to work" where staff can view their schedules. This is monitored and facility can see viewing for when staff reviews their schedules. You can also pick up shifts on this app as well. Will need 2-3 days with approval prior to seeing on app. Additional policies: - Offer education CEU hours (some off site) for all staff. Not mandatory. - Nurses: monthly staff meetings. 80/20 attendance needed. Will be posted in several locations when meetings will be held. Nurse Responsibilities pertaining to Wonder-guard Checks: The following must be conducted daily, we will have this added into the resident?s TAR for documentation purposes. 1. Check placement of wonder-guard every shift 2. Once a day ensure wonder-guard is properly working. 3. Immediately replace wonder-guard if missing or not working a. Inform on-call nurse if there are any issues
