RN – Long-Term Care / Geriatric Nursing

Required Skills / Certifications / Experience

• Minimum 2 years of nursing experience required.
• First-time travelers accepted with strong, relevant clinical experience.
• Local candidates accepted; rate may vary.
• Previous Long-Term Acute Care (LTAC) experience required.
• PointClickCare experience required.
• BLS certification required.
• Strong experience with geriatric, nursing home, and long-term care patients.
• Proficient in patient assessment, medication administration, IV therapy, venipuncture, blood glucose monitoring, insulin administration, respiratory care, wound care, catheter care, ostomy care, feeding tube management, and infection control.
• Ability to perform neurological, respiratory, skin, pain, fall-risk, and safety assessments.
• Experienced with oxygen therapy, nebulizer treatments, BiPAP/CPAP, pulse oximetry, seizure precautions, and emergency procedures.
• Skilled in patient transfers, intake/output monitoring, narcotic counts, isolation precautions, PPE, and pressure-ulcer prevention.

Preferred Skills / Experience

• Experience in skilled nursing or rehabilitation centers preferred.
• Experience with nephrostomy tubes, suprapubic catheters, PICC/central lines, and peripheral IVs.
• Experience caring for patients with Alzheimer’s, dementia, CVA/stroke, Parkinson’s, neuromuscular disorders, spinal cord injuries, TBI, and amputations.
• Knowledge of wound assessment, pressure-ulcer staging, Wound VACs, suture/staple removal, and specialty beds.
• Experience with tracheostomy care, suctioning, and complex respiratory needs preferred.
• Experience with admissions, discharges, care plans, patient/family education, and end-of-life/palliative care.
• Previous charge nurse experience preferred.
• Community hospital and teaching hospital experience preferred.
• Trauma Level I or Level II experience preferred.

• Unit Census: Approximately 36 patients / 40 beds
• Unit Support: 2 CNAs
• MD Coverage: On-call physician available after hours

Staffing & Patient Ratios

• Standard night staffing ratio: 1 RN : 40 residents
• Weekend staffing ratio: 1 RN : 40 residents
• Actual patient assignment is typically 1:20.
• From 7:00 PM–11:00 PM, two nurses are on the unit, with approximately 20 residents per nurse during the heavier medication pass.
• After 11:00 PM, staffing decreases to one nurse on the unit.
• Unit acuity is generally low, with limited IV therapy and a small number of residents requiring higher-level nursing interventions.

Patient Population

• Primarily geriatric residents.
• Common care needs include Foley catheters, G-tubes, blood glucose monitoring, medication administration, wound/skin care, and chronic disease management.

Orientation

• 24-hour unit orientation.
• Hospital orientation is conducted during Week 1.
• Traveler must be available to attend required orientation classes each day during the first week.
