- Weekend Requirement: Every other weekend
- Holiday Requirement: Required
Requirements
- Active RN License
- Experience in Post Acute/Skilled Nursing preferred
- Ability to work nights, weekends, and holidays
Required Experience
- Post Acute Care
- Long-Term Care/Skilled Nursing (preferred)
Required Certifications
- BLS
- Active RN License
Key Responsibilities
- Provide direct nursing care to post-acute patients.
- Assess, implement, and evaluate individualized patient care plans.
- Administer medications and treatments.
- Document patient care in PointClickCare (PCC).
- Collaborate with the interdisciplinary care team.
- Maintain compliance with facility policies and patient safety standards.
