Assess and monitor residents’ medical conditions and document changes promptly.
Administer medications and treatments according to physician orders and facility policies.
Develop, implement, and update care plans based on resident needs and health status.
Provide wound care, dressing changes, catheter care, and chronic disease management.
Supervise and support CNA/LPN staff, ensuring quality and consistency in resident care.
Coordinate with physicians, therapy teams, and families to optimize resident outcomes.
Assist with activities of daily living support (ADLs) as needed while promoting independence.
Ensure accurate documentation in EMR and maintain clear communication across shifts.
Monitor for signs of infection, falls, and safety concerns, and initiate appropriate interventions.
Respond to emergencies and provide stabilization until higher care can be arranged if needed.
Participate in care conferences and facility compliance activities (state surveys, audits, etc.).
Maintain compassionate resident-centered care with dignity, empathy, and patience.
