• Schedule: 8-hour day shifts
• Hours Per Week: 40
• Weekend Rotation: Every 4th weekend
• On-Call: Every 4th week
• Estimated Weekly Pay: $1,880 - $1,980
• Travel Territory: Up to 75 miles from the assigned care center
• Charting: HomeCare HomeBase (HCHB); OASIS proficiency required for RNs

Job Requirements

• Minimum 1 year of Adult Home Health experience or 1 year of Hospice experience, depending on specialty
• Active Registered Nurse license in hand
• Current CPR/BLS certification
• Recent experience using HomeCare HomeBase within the last 6 months, or willingness to complete retraining if needed
• Recent OASIS proficiency within the last 6 months, or willingness to complete retraining if needed
• Reliable transportation, valid driver's license, and auto insurance
• Comfortable traveling between patient homes throughout the day and attending weekly meetings
• Ability to work independently in a home health environment
• Open to weekend and on-call rotation as scheduled

Responsibilities

• Provide skilled nursing care in patients' homes according to physician orders and care plans
• Perform assessments, start-of-care visits, wound care, follow-up visits, and other assigned home health services
• Develop, coordinate, and direct home nursing care in alignment with agency policies and procedures
• Supervise and evaluate home health aides and licensed practical/vocational nurses as applicable
• Document all patient care accurately and within 24 hours of each visit
• Manage weekly productivity expectations and patient visit schedules
• Travel efficiently between patient homes and complete visits across the assigned territory
• Participate in periodic office meetings and maintain clear communication with the clinical team
