Job Requirements

• Active Registered Nurse (RN) license in hand
• Minimum 1 year of adult home health experience or 1 year of hospice experience
• Recent specialty experience required
• Current CPR/BLS certification
• Proficiency with HomeCare HomeBase EMR within the last 6 months
• OASIS proficiency required for RN candidates
• Reliable transportation, valid driver’s license, and active auto insurance
• Comfortable driving to multiple patient homes daily and attending weekly office meetings
• Willingness to complete documentation within 24 hours of each visit
• Open to on-call rotations and variable weekend coverage as needed

Responsibilities

• Provide skilled nursing care to patients in their homes based on physician orders and agency guidelines
• Coordinate, direct, and document home health services using the nursing process
• Develop and follow individualized care plans to support patient recovery, safety, and continuity of care
• Complete visits, travel, and documentation efficiently while maintaining high-quality patient care
• Assess patient needs and respond appropriately to changes in condition
• Supervise and evaluate home health aides and LPN/LVNs as assigned
• Manage a varied caseload that may include start-of-care, wound care, and follow-up visits
• Maintain punctual, accurate charting and strong communication with the interdisciplinary care team
• Travel throughout the assigned territory and remain flexible to patient scheduling needs
