Job Requirements

• Active Registered Nurse (RN) license in New York State
• Minimum of 1 year of nursing experience preferred; home health experience strongly preferred
• Associate degree in Nursing or higher
• Professional certification required
• Strong clinical assessment and documentation skills
• Ability to work independently in a home care environment
• Excellent communication and patient education skills
• Experience with wound care and IV management is highly desirable

Responsibilities

• Provide skilled nursing care to patients in their homes
• Perform wound care, IV management, and other ordered nursing interventions
• Deliver patient and family education to support safe care at home
• Assess patient conditions, monitor progress, and report changes as needed
• Coordinate care within the home health team to help ensure quality outcomes
• Document visits accurately and maintain timely clinical records
• Travel within the assigned coverage area to complete patient visits
• Follow established care plans while promoting dignity, comfort, and independence
