Job Requirements

• Active RN license in Tennessee or a compact state license
• Experience with HCHB (Home Care Home Base) required
• Hospice nursing experience required
• Associate degree required
• Strong communication skills, flexibility, and a collaborative approach to patient care
• Professional certification required

Responsibilities

• Provide hospice RN case management in a homecare and home health setting
• Support admissions and death visits as needed
• Prepare for and participate in IDT meetings
• Deliver compassionate, patient-centered care in the home
• Document and manage care plans accurately using HCHB
• Work within a coverage area spanning Shelby, Fayette, and Tipton Counties
• Adapt to changing patient needs while maintaining high-quality clinical care
