Job Requirements

• Active Arkansas Registered Nurse (RN) license
• Current CPR certification
• Valid Arkansas driver's license and vehicle insurance
• Associate degree required
• Home health nursing experience strongly preferred
• OASIS experience preferred
• HCHB experience preferred
• Strong documentation, communication, and time management skills
• Ability to work independently while collaborating with an interdisciplinary care team

Responsibilities

• Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and other healthcare professionals
• Provide skilled nursing visits in the home setting in accordance with the plan of care and infection control standards
• Complete comprehensive assessments, including OASIS and medication reconciliation, accurately and on time
• Document patient visits thoroughly and submit records according to payer and agency requirements
• Perform initial and/or comprehensive nursing evaluations and confirm home health eligibility and medical necessity
• Communicate effectively with physicians, patients, families, and agency staff to coordinate care and schedule changes
• Supervise home health aides and licensed practical/vocational nurses as appropriate and complete supervisory visits
• Participate in weekend rotation, holiday coverage as needed, case conferences, and quality improvement efforts
• Support patient safety, reduce unnecessary hospitalizations, and promote positive patient outcomes
• Assist with onboarding and serve as a preceptor for new staff and students when needed
