Job Requirements

• Minimum of 1 year of nursing experience in home health, homecare, or a related clinical setting
• Associate degree required
• Current CPR certification required
• Strong home health care experience preferred
• Ability to perform independent patient assessments and manage a caseload in the home setting
• Excellent communication, time management, and documentation skills
• Experience with OASIS competency preferred
• Helpful skills include PICC line management, indwelling catheter management, new ostomy teaching, and complex wound care

Responsibilities

• Serve as a case manager for assigned home health patients
• Complete routine home visits and comprehensive physical assessments
• Develop, implement, and follow individualized patient care plans
• Document visits accurately and maintain strong clinical records
• Support patients and families with education, guidance, and compassionate care
• Coordinate care across the assigned coverage area and communicate effectively with the care team
• Provide skilled nursing support while maintaining quality, safety, and continuity of care
