Job Requirements
- Active Registered Nurse (RN) license
- Associate degree in Nursing required
- At least 1 year of nursing experience required
- Previous home health nursing experience required
- Ability to participate in on-call coverage as scheduled
- Professional, dependable, and comfortable working independently in a field-based environment
- Preferred clinical skills: Pleurex care, central line care, Foley and suprapubic insertion, wound care, wound vacs, and venipuncture
- Ability to follow care plans, document accurately, and provide patient-centered support in the home setting
Responsibilities
- Deliver skilled nursing care to patients in the home health setting
- Complete patient visits, assessments, and ongoing monitoring based on individualized care plans
- Provide education and support to patients and families to promote safe, effective home care
- Perform clinically appropriate nursing interventions within scope of practice
- Support wound care, catheter care, line care, and other specialized treatment needs as assigned
- Document patient status, interventions, and outcomes accurately and in a timely manner
- Coordinate with the interdisciplinary care team to help ensure continuity of care and positive patient outcomes
- Participate in scheduled on-call coverage and respond professionally to patient needs after hours
