Registered Nurse Requirements

• Active Registered Nurse (RN) license
• Minimum 1 year of nursing experience required; home health or hospice experience preferred
• Strong clinical assessment, care coordination, and patient education skills
• Ability to work independently while collaborating with physicians and interdisciplinary team members
• Experience with case management or lead discipline responsibilities is a plus
• Ability to adapt to a rotating schedule and provide reliable weekend coverage as assigned

Home Health RN Responsibilities

• Deliver individualized nursing care using the nursing process in accordance with physician orders
• Perform comprehensive patient assessments and develop appropriate care plans
• Serve as case manager when acting as the lead discipline
• Coordinate care across disciplines to support safe, effective patient outcomes
• Educate patients and families on treatment plans, safety, and self-care
• Document patient care accurately and maintain timely clinical records
• Support a high standard of compassionate, community-based home health and hospice care
