• Float Coverage: Upper Cape area, with occasional floating to Mid Cape

RN Job Requirements

• Active Massachusetts RN license
• Minimum 1 year of nursing experience required, or 6 months of nursing experience with a BSN
• Current CPR certification required
• Successful completion of pre-hire medication test
• Home health nursing and IV experience preferred
• Strong computer and clinical documentation skills preferred
• BSN preferred

Home Health RN Responsibilities

• Complete comprehensive nursing assessments at admission
• Develop, implement, and revise individualized plans of care
• Use clinical judgment to determine visit frequency and coordinate interdisciplinary care
• Monitor patient condition and notify providers of significant changes
• Participate in interdisciplinary case conferences and evaluate plan-of-care effectiveness
• Provide and document supervision of Home Health Aide care every 14 days
• Complete admissions, skilled visit documentation, and discharge paperwork accurately and on time
• Obtain and clarify valid provider orders for all care delivered
• Educate patients and caregivers while documenting skilled nursing care clearly and thoroughly
• Coordinate safe discharges and connect patients with community resources as appropriate
