This role focuses on creating a caring environment aimed at healing and wellness. Nursing practices are based on evidence and current research, as well as hospital and department policies and procedures. Regulatory standards and professional guidelines are also considered in practice. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit. This includes the provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also part of the role. Provision of patient and family-centered education is essential. Care coordination and individualized treatment planning may be applicable. The RN assesses patient needs and performs comprehensive assessments in accordance with their scope of practice. Consideration for the patient’s culture, age, and developmental level is required. Identifying patient risk factors and presenting complaints is part of the assessment process. Obtaining patient health information from family or others as necessary is expected. Objective and subjective data is collected as part of the assessment. Verifying initial admission assessments obtained by other team members is important. A prioritized problem list is developed based on input from the inter-professional team. Actions and interventions relevant to patient culture and age are designed accordingly. Desired outcomes are determined in coordination with patients and families as per the medical plan of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy. Explaining procedures, treatments, and processes to patients and families is required. The RN takes action to alleviate patient stress and discomfort. Recognizing, responding to, and communicating changes in patient condition appropriately is essential. Ongoing assessments and responses to procedures and therapies are documented according to policy and standards. Tasks are delegated appropriately, and the RN oversees team members in the performance of patient care activities. The accuracy and completeness of delegated patient care by team members is monitored. Educational needs of patients related to prioritized problems are identified and addressed. The RN facilitates the discharge process by executing orders timely and performing discharge teaching. Providing input into patient/nurse assignments considering available staffing and patient acuity is expected. Active participation in unit-based activities supporting organizational values and priorities is encouraged.
