Delivers coordinated nursing care for a patient or an assigned group of patients according to established standards of care and the nursing process. Supervises and directs the activities of various levels of assigned nursing staff. Coordinates care with other disciplines while utilizing critical thinking and professional discretion. Job responsibilities include: Collecting relevant data pertinent to the patient's health or situation. Analyzing assessment data to determine diagnosis and care issues. Developing a plan that prescribes interventions to attain outcomes. Implementing the plan, coordinating care delivery, and employing strategies to promote health and safety. Evaluating progress toward attaining outcomes. Identifying outcomes for the patient or the patient's situation. Collaborating with the patient, family, and healthcare providers in providing patient care in a safe and caring environment. Providing learning opportunities for patients, family members, and team members. Directly providing health information to patients, families, and treatment teams. Participating in discharge planning to provide continuity of care. Delegating appropriately and coordinating duties of healthcare team members. Performing other job-related duties as assigned. This role supports the delivery of intensive, comprehensive care and monitoring for medical, surgical, and cardiac patients experiencing life-threatening conditions. This unit serves a mix of medical, surgical, and cardiac intensive patients. Responsibilities may include ensuring the safety and well-being of patients and associates, including compliance with vaccination requirements.
