Works in controlled patient care situations. Performs established nursing care procedures. Engages in therapeutic, rehabilitative, and preventive activities under the direction of a registered nurse. Provides care to patients ranging in age from infant to geriatric. Makes decisions based on standard practice, instructions, operating directives, and established procedures. May be required, with orientation, to provide service and/or patient care to other areas. Collaborates in the assessment of patients and their significant others. Performs patient assessments as indicated by the patient's condition or as directed. Notifies the physician of admission and obtains admitting orders per departmental policy. Gathers data to be used in assessments for each patient every 8 hours or more frequently as indicated. Includes information obtained from the patient and significant others in the data gathering process. Gathers data regarding needs and learning abilities of the patient during admission and thereafter. Assists with identifying patient problems. Collaborates in the development of an outcome-oriented plan of care. Plan of care should include identified problems and/or nursing diagnoses, nursing interventions, and goals/outcomes with target dates. Reviews the plan of care for assigned patients every 8 hours and collaborates on revisions as necessary. Involves the patient and/or significant others in decisions regarding care. Prioritizes care delivery according to patients' needs. Demonstrates the ability to interpret information in terms understandable to the patient. Assists in planning care based on knowledge of growth and developmental changes occurring across the patient’s life-span. Implements the plan of care under the direction of the registered nurse. Provides holistic nursing care to an assigned group of patients. Assists with and/or performs procedures. Provides education based on addressed needs including the safe use of equipment used in treatment. Demonstrates the ability to provide care and information based on physical, psychosocial, cultural, safety, and other relevant factors. Demonstrates knowledge of medications and their correct administration based on the age of the patient and clinical condition. Follows the five medication rights and reduces the potential for medication errors. Assists in evaluating the effectiveness of services provided. Collaborates in evaluating patient responses to care and in determining progress toward achievement of outcomes. Evaluates patient understanding of information and effectiveness of discharge plans. Collaborates in obtaining data on patient satisfaction. Demonstrates effective and accurate verbal, written, and electronic communication. Creates a positive climate for communication and functions as a team player. Maintains confidentiality of patient, staff, and hospital information. Communication is timely, accurate, concise, complete, and appropriate. Completes documentation of care and other pertinent data according to policies. Treats patients, family members, and staff with respect while maintaining a positive work environment. Participates in unit and organizational activities. Participates in a significant percentage of staff meetings and Performance Improvement activities. Participates in a committee or special project as requested. Complies with organizational policies and procedures. Maintains current certification in CPR and attends all mandatory in-services. Reports to work on time and adheres to the dress code. Completes required competencies on a yearly basis. Maintains hospital property and the hospital environment in a safe and clean condition. Nursing documentation is completed by the end of the shift, with exceptions limited to rare circumstances. Demonstrates continued development of knowledge and skills. Participates in unit development and decision making through activities such as attending meetings and serving on committees. Demonstrates the knowledge and skills to provide care based on the physical, psychosocial, educational, safety, and other related criteria appropriate to all age groups. Utilizes knowledge of growth and development in all patient care activities. Interprets age-specific responses to therapies and treatments. Involves families and/or significant others in decision-making related to the plan of care. Assists with answering call lights and following through with requests. If requests are not within the scope of practice, notifies the registered nurse at the time of the request. Demonstrates professional conduct and development. Able to learn and react quickly in performance of multiple tasks. Competent to make independent decisions within the realm of licensure and seeks assistance as required. Functions under the direction of the registered nurse and follows unit guidelines. Effective interpersonal skills are required for interacting with patients, staff, and visitors.
