The role requires independent judgment to provide effective patient care.
Data pertinent to the patient’s health or situation is collected and analyzed.
Assessment data helps in determining diagnosis and care issues.
A plan is developed that prescribes interventions to achieve desired outcomes.
Implementation of the plan involves coordinating care delivery and employing strategies to promote health.
Progress toward outcome attainment is regularly evaluated.
Outcomes for the patient’s situation are identified.
Collaboration with the patient, family, and healthcare providers is essential.
Nurses provide learning opportunities for patients, families, and team members.
Health information is directly communicated to patients and families.
Participation in discharge planning ensures continuity of care is provided.
Duties of healthcare team members are delegated appropriately.
Other job-related duties may be assigned as necessary.
