Portland, ME | Skilled Nursing Facility / Transitional Care

Schedule / Job Type

Contract | 13 Weeks | Night Shift (7:00 PM – 7:30 AM) | Onsite | Expected 48 hours/week preferred, 36 hours/week acceptable | Every other weekend required

Key Responsibilities

• Provide direct patient care on a 46-bed Transitional Care / Medical-Surgical unit serving a mix of medical and surgical patients who are medically ready for discharge
• Maintain patient ratios of up to 1:8, delivering patient-centered, high-quality nursing care
• Utilize Epic EMR for accurate and timely clinical documentation
• Operate and manage unit equipment including specialty beds, Philips monitors, overhead lifts, bladder scanners, and other assistive devices
• Demonstrate charge nurse experience and provide leadership support as needed on the unit
• May be floated to other units based on skillset and facility need

Required Qualifications

• Active RN licensure (or compact eligibility) in the state of Maine
• BLS certification required
• Minimum 1–2 years of recent acute care nursing experience
• Proficiency with charting and EMR systems (Epic preferred)
• Completion of required Healthstream education modules prior to or upon start
• Charge nurse experience required

Preferred Experience / Skills

• Experience in a Transitional Care, LTC, or SNF setting
• Familiarity with specialty equipment such as Synergy Air beds, Sara Steady lift systems, and Hover/Sage mats
• Strong interpersonal and communication skills with a collaborative, team-oriented approach
• Demonstrated reliability, punctuality, and commitment to patient-centered care
