Evaluating progress toward achieving outcomes and identifying outcomes for patients are integral tasks. Collaboration with patients, families, and healthcare providers in a safe and caring environment is essential. Providing learning opportunities for patients and team members is part of the job. Directly providing health information to patients and families is expected. Participation in discharge planning to ensure continuity of care is required. Delegating appropriately and coordinating duties of healthcare team members are essential functions. Other job-related duties may be assigned as necessary.
