Provides safe, competent quality care based on nursing theory and research. Delivers patient care by incorporating the professional practice model of Relationship Centered Care. Applies the nursing process based on theory, research, evidence-based practice, and nursing standards. Assesses and evaluates patient care needs. Applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes patient care aspects including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and team members. Manages assignments within the Care Delivery Model of Modified Primary Nursing. Role models the tenets of Relationship Centered Care. Considers patient values, preferences, and cultural diversity in care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patients and families with concerns.
