Job Summary:
The Oncology Nurse Navigator position is based in Palo Alto at a prominent healthcare facility. This role involves coordinating cancer clinical care, providing patient education, and addressing both clinical and psychosocial needs throughout the patients cancer journey. The nurse navigator works closely with interdisciplinary teams to ensure holistic care and support shared decision-making, ultimately enhancing patient outcomes and experience.

- Required Qualifications: Bachelors Degree of Science in Nursing from an accredited college or university; CA-RN (Registered Nurse) license; minimum of 0 years of experience; clinical knowledge of disease processes, symptom management, and cancer care services; strong communication and interpersonal skills; experience with MS Office.
- Preferred Qualifications: MS or MSN degree; 2-3 years of oncology nursing experience; nurse navigation and ambulatory oncology case management experience; oncology nurse certification (OCN) preferred; experience with Epic EMR; knowledge of adult learning styles and educational program delivery; ability to develop materials involving graphics and tables; BLS certification; AOCNS (Advanced Oncology Certified Clinical Nurse Specialist) preferred.
- Additional Information: This role includes responsibilities such as assessing patient education needs, facilitating communication within care teams, addressing barriers to care, and supporting accreditation standards within the cancer service line. The nurse will serve as a clinical advocate and liaison, providing culturally appropriate education, screening for distress, and connecting patients to community support services.
