Description:
The position requires the completion of various personal health assessments and vaccinations.
These include a Mask Fit Test and several immunizations such as MMR, COVID-19 Vaccine, Flu Vaccine, Tdap, Varicella, and Hepatitis B.
A Drug Screen and TB Questionnaire will also be mandatory.
Candidates must submit an Employee Medical History Form and a Health Statement.
Human resources documentation must include a signed job description, a signed offer letter, and a background check.
Additional certifications required are BLS and ACLS, as well as completion of an Annual TRA Evaluation.
Candidates must also complete a Unit Knowledge Exam and other assessments related to operating room skills.
It is essential to have a Driver's License or State ID.
