A problem list is developed, prioritizing patient needs based on input from the inter-professional team.
Actions and interventions are designed according to the patient's culture and age.
Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care.
Patient care therapies and procedures are fully implemented within the scope of practice and established guidelines.
The RN explains procedures, treatments, and processes to the patient and family.
Initiatives are taken to alleviate patient stress and discomfort.
Changes in patient condition are recognized, responded to, and communicated appropriately.
Ongoing assessments and responses to procedures and therapies are conducted and documented.
Tasks are delegated appropriately, and oversight is provided to team members.
