Key Responsibilities

• Provide skilled home health nursing care to patients across an assigned county territory, with a primary focus on the Shelton area and surrounding towns including Ansonia, Derby, Seymour, and Oxford (New Haven County)
• Perform comprehensive patient assessments and deliver care in accordance with individualized care plans
• Provide and manage wound care as a core clinical responsibility
• Document patient visits and clinical notes using the PointCare EMR system
• Meet productivity expectations as a Primary Care Nurse, with an expected output of 28 units per week within a 40-hour work week
• Complete orientation in the first week with an expectation of full productivity by the second week

Required Qualifications

• Active and unrestricted Registered Nurse (RN) license in the state of Connecticut
• Prior home health nursing experience required
• Wound care experience required
• Ability to work independently in a field-based, home visit environment
• Fingerprinting is required for placement
• No on-call requirement

Preferred Experience / Skills

• Familiarity with PointCare or similar home health EMR platforms
• Experience managing a caseload across multiple towns or geographic territories
• Strong time management and self-directed work habits suited to a field-based role
