**Exciting Opportunity: Clinical Care Registered Nurse - Kalamazoo M-F 8:00 AM-4:30 PM**
**Job Description:**
- **Primary Purpose:**
As a Clinical Care Registered Nurse (CCRN), you will provide direct nursing care to participants, mastering the art of nursing in a dynamic environment. This role empowers you to work independently using your advanced nursing knowledge and skills to manage complex clinical situations. You will collaborate with both licensed and non-licensed personnel to ensure optimal patient outcomes.
- **Key Responsibilities:**
- Triage emergent situations with confidence and expertise.
- Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
- Utilize care plans independently to deliver exceptional clinic and home-based direct care.
- Administer treatments, procedures, and medications as prescribed to participants.
- Conduct regular home visits to provide care and treatment ordered by PCPs, including wound care, lab draws, nurse assessments, and other nursing tasks.
- **Qualifications:**
- Bachelor’s degree preferred in Science of Nursing; Associates degree required.
- RN–Nursing Diploma or Degree from an accredited School of Nursing or University.
- Minimum of one year of experience working with the frail, elderly, or long-term care population.
- Minimum of one year home care experience preferred.
- Minimum of one-year wound care experience preferred.
- **Specific Requirements:**
- Current CPR certification by AHA or willingness to obtain as provided by Senior Care Partners PACE.

- **Work Environment:**
- Maintain visible presence while working in the center and adapt to frequent interruptions.
- Communicate effectively with staff, participants, caregivers, and other organizations as necessary.
- Flexibility to work beyond normal hours and on weekends and holidays as needed.
- **Minimum Working Requirements:**
- Reliable transportation, valid Michigan driver’s license, and car insurance.
- Must be 18 years of age or older.
- Must be fully vaccinated, or willing to be fully vaccinated against various communicable diseases.
**Shift Information:**
M-F 8:00 AM - 4:30 PM - on-call/holiday rotations
