Job Summary Transitional Care Unit (TCU) Nurse Torrance, CA This role involves providing specialized nursing care in a sub-acute rehabilitation setting within a skilled nursing facility. The position focuses on rehabilitative care for adult and geriatric patients with a variety of diagnoses, supporting their recovery and functional improvement in a dedicated transitional care environment.
Job Details
Position Description: REHAB/SUB ACUTE on TCU *Skilled Nursing facility covers all system diagnosis and follows certain admit criteria. No custodial / long term care. *TCU is a SNF and therefore the average nurse to patient ratio is 8:1. *TCC/TCU is a distinct part of LCM - sub acute care and has an 87 bed capacity and are focused on rehabilitation of patient. *The TCU unit is located across from the main Hospital - 4320 Maricopa St, Torrance, CA 90503.
Certifications: BCLS
Schedule: Nights shift, 12 hours, minimum of 36 hours per week.
Required Qualifications: Valid licensure in the state, at least 1 year of experience in a transitional care or similar setting, certification in BCLS, and experience with electronic charting systems.
Preferred Qualifications: Experience in a community hospital or sub-acute rehabilitation setting.
Additional Information:
Must be able to provide references covering the last 3 years.
Candidates should be prepared to work every other weekend.
Float rotation may be required based on facility needs.
Candidates must have current, valid state licensure or a compact license at the time of submission.
RTO (Reasons To Occur) must be included with submission, and all requests are subject to approval.
Candidate responses to specific pre-screening questions regarding previous work at Providence facilities and references are required upon submission.
