This role is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. The nurse will participate in identifying and meeting their own learning needs. Attendance at education programs based on identified learning needs is required. The nurse will assess the readiness to learn of patients, families, and caregivers. They will evaluate care by identifying expected patient outcomes. The nurse will seek guidance as needed in revising the plan of care. Participation in discussions with the interdisciplinary team regarding the evaluation of patient care is essential. Effective communication of relevant information to promote continuity of care is expected. Demonstrating competence in the care of patients with complex problems is required. The nurse will perform bedside point of care testing as required for the patient care plan. Utilizing healthcare organization and nursing standards, policies, and procedures in the delivery of care is essential. Organizing and prioritizing care according to patient and family needs is necessary. The nurse will consult with appropriate resources regarding patients with complex care issues. With assistance, the nurse will develop goals to promote professional growth. Achieving goals and objectives within identified time frames or renegotiating with supervisors is expected. Knowledge of activities that facilitate interdepartmental collaboration is essential. Participation in unit goals and performance improvement activities is required. The nurse will effectively use communication systems. Promoting cost-effective care is essential. Feedback to co-workers is encouraged. Awareness of public policy and regulatory guidelines affecting the healthcare environment is required. Promoting a safe, clean, and secure hospital environment for all is essential. Recognizing data from complex situations to determine care priorities is expected. Assessment of physical, psychosocial, educational, and safety needs is required. The nurse will synthesize assessment data into meaningful whole prior to communication to others. Assessment and anticipation of discharge needs for individual patients and families is necessary. Developing relationships with families to empower them to advocate for the patient is essential. Collaboration with patients and families to prepare or update the plan of care is required. Utilization of available multidisciplinary resources in planning care is expected. The nurse will be able to identify and address cultural and ethnic issues in patient care. Demonstrating the ability to prioritize tasks for patients with complex problems is required. Reading relevant journals and articles to enhance practice is expected.
