The RN performs and documents a comprehensive assessment in accordance with scope of practice and hospital policies.
Consideration is given to the patient’s culture, age, and developmental level.
Identifying patient risk factors and presenting complaints is crucial.
The RN collects objective and subjective data as needed.
Initial admission assessments are verified if obtained by other team members.
A problem list is developed, prioritizing patient needs.
The RN designs actions and interventions relevant to the patient's culture and age.
Desired outcomes are determined in coordination with the patient and family, in accordance with the medical plan of care.
Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy.
Explanations of procedures, treatments, and processes are provided to patients and families.
Initial actions to alleviate the patient’s stress and discomfort are initiated.
Changes in patient condition are recognized, responded to, and communicated appropriately.
Ongoing assessments and responses to procedures are documented according to hospital policy and established standards.
The RN delegates tasks appropriately and oversees team members in performing patient care activities.
Accuracy and completeness of delegated patient care are monitored.
Educational needs of patients related to prioritized problems are identified and addressed.
The discharge process is facilitated by executing discharge orders in a timely manner.
Input into patient/nurse assignments is provided considering staffing and patient acuity.
