About the Position

We are seeking an experienced Home Health RN with a strong background in psychiatry/behavioral health to provide patient-centered care across Orange and Los Angeles Counties.
In this role, you will:

• Conduct start-of-care assessments, follow-ups, and discharges for home health patients with behavioral health needs.

• Provide psychological assessments and interventions, collaborating closely with patients, families, and interdisciplinary teams.

• Deliver compassionate, evidence-based care that promotes patient independence and wellness in the home setting.

• Travel within a 20–30 mile radius of your home base to see patients.

Requirements

• Active RN license (required)
• Psychiatric/Behavioral Health experience (required)
• Home Health experience (preferred, strongly considered)
• Proficiency in case management and care coordination
• Strong communication and patient education skills
• Ability to work effectively within a multidisciplinary team
• Current CPR certification
