Utilizes the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs while applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in developing, implementing, and evaluating the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning post-hospital care. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing and allocates resources effectively. Role models principles of Relationship Centered Care during patient interactions. Considers patients' values, preferences, and cultural diversity in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Utilizes evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and families who express concerns. Demonstrates caring, respect, compassion, empathy, and active listening in all interactions. Conducts purposeful hourly rounding, addressing the four P’s of patient care. Practices safe and efficient patient-centered care, identifying and escalating barriers to improvement. Demonstrates essential clinical skills and knowledge to ensure quality and safety in the patient population served. Implements process improvement strategies following institutional evidence-based procedures. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and nursing research. Adheres to quality and safety compliance measures. Identifies and mitigates potential safety hazards following established protocols. Documents nursing practice accurately to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with leadership to ensure adequate supply utilization and inventory for patient care. Escalates resource deficits as needed. Documents in compliance with care standards and regulatory requirements. Facilitates patient throughput during shifts. Contributes to a supportive environment for clinical colleagues, patients, and their families. Advances clinical competence to progress from novice to expert. Projects a professional image and communicates with respect and empathy. Incorporates ethical principles into decision-making for patient and family care. Encourages and demonstrates lifelong learning and innovation. Participates in shared decision-making through councils and unit initiatives. Experience with Epic preferred. Average unit census is 21-25 beds.
