Job Title: Registered Nurse Profession: Nursing Specialty: Rehabilitation Duration: 13 weeks Shift: Night Shift Hours per Shift: 12.5 hours Experience: Minimum 2 years RN experience; 2 years of rehabilitation and mechanical ventilation experience License: NYS RN License Certifications: BLS Must-Have: - Excellent Organizational Skills - BLS Card Description: Delivers patient care by incorporating the principles of Relationship Centered Care. Applies the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs and utilizes critical thinking skills effectively. Integrates relevant assessment and intervention skills in nursing care delivery. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the care team. Manages assignments within the Care Delivery Model and allocates resources effectively. Role models the tenets of Relationship Centered Care through various care facilitation actions. Considers patient values, preferences, and cultural diversity in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patients and family members with concerns. Demonstrates compassion, empathy, and active listening through dialogue and actions. Supports staff in utilizing evidence-based practices to enhance understanding of patient care. Conducts Purposeful Hourly Rounding on assigned patients. Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial for quality and safety within the patient population. Implements process improvement strategies based on evidence-based procedures. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects. Contributes to quality and safety practices and required compliance measures. Implements high reliability principles to identify and avoid high-risk behaviors. Escalates potential safety hazards or best practice gaps through the established protocol. Documents nursing practice to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with management to ensure appropriate utilization of supplies and inventory. Escalates material or staffing resource deficits as needed. Demonstrates patient-focused and cost-effective approaches to care. Documents accurately and thoroughly in compliance with standards of care. Facilitates patient throughput on a shift basis. Contributes to the environment of care to support colleagues, patients, and families. Advances clinical competence in nursing practice to progress from novice to expert. Projects a professional image and communicates with care and respect. Incorporates ethical principles into decision-making for patients and families. Encourages scholarship, inquiry, and lifelong learning. Acquires knowledge and skills relevant to the role and patient population. Participates in shared decision-making through various committees and initiatives.
