Job Title: Clinical Nurse Profession: RN Specialty: Telemetry Duration: 9 Shift: Night Shift Hours per Shift: 12 Experience: Minimum 2 years RN experience; 2 years telemetry and critical care experience preferred License: NYS RN License Certifications: BLS, ACLS, Tele, Ventilator Must-Have: Advanced Cardiac Life Support (ACLS) Description: The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care to a designated group of patients and their significant others. Delivers patient care by applying nursing theories, research, and evidence-based practices. Assesses and evaluates patient care needs and utilizes critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in developing, implementing, and evaluating the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning post-hospital care, delegating to others as appropriate. Educates patients and caregivers about their plan of care, health promotion, and disease prevention while anticipating their needs and readiness to learn. Communicates effectively and professionally with patients, families, and all members of the healthcare team. Manages assignments within the patient care delivery model and demonstrates effective allocation of resources. Role models relationship-centered care through established practices and patient interactions. Considers the patients' values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions. Initiates service recovery for patients and family members who have concerns, escalating them as needed. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue and actions. Conducts hourly rounding on assigned patients addressing their needs. Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge essential to quality and safety in the patient population served. Implements process improvement strategies based on evidence-based practices and protocols. Engages in peer review processes and collaborates with teams to create and implement quality improvement projects. Participates in quality and safety practices and compliance measures. Utilizes institutional information technology by documenting nursing practice to support improvement initiatives. Performs charge nurse duties as assigned and coordinates with nursing leadership to ensure effective resource utilization. Documents in an accurate and thorough manner in compliance with regulatory and legal requirements. Facilitates patient throughput on a shift basis. Advances clinical competence to progress from novice to expert in nursing practice. Projects a professional image and communicates with respect and compassion. Incorporates ethical principles into decision-making for patients and families. Encourages a spirit of inquiry and lifelong learning for self and others. Participates in shared decision-making through nursing committees and unit initiatives.
