Delivers patient care by applying nursing theories, research, and evidence-based practices. Assesses and evaluates patient care needs and utilizes critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in developing, implementing, and evaluating the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning post-hospital care, delegating to others as appropriate. Educates patients and caregivers about their plan of care, health promotion, and disease prevention while anticipating their needs and readiness to learn. Communicates effectively and professionally with patients, families, and all members of the healthcare team. Manages assignments within the patient care delivery model and demonstrates effective allocation of resources. Role models relationship-centered care through established practices and patient interactions.
