Responsibilities- Collect complete health history from patient, family, and medical records.
- Perform and document admission assessments within required timelines.
- Complete Braden Scale accurately and reassess patient needs regularly.
- Develop individualized, age‑appropriate plans of care for adolescent, adult, and geriatric patients.
- Update and revise care plans based on changes in patient condition.
- Prioritize and perform direct patient care safely and effectively.
- Delegate tasks appropriately according to caregiver skill level.
- Coordinate care using an interdisciplinary approach.
