Job Title: Heart Transplant Coordinator Profession: Registered Nurse Specialty: Transplant Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 15:30) Experience: Minimum 2 years of relevant experience License: Active Registered Nurse License Certifications: BLS, ACLS, Bachelor's Degree Must-Have: Experience with transplant populations Description: Coordinates all aspects of care, both inpatient and outpatient. Responsibilities include record review, medication refills, and prior authorizations. Provides patient education and reviews test, procedure, and lab results. Makes referrals for medically necessary services, consults, equipment, and supplies. Functions as a liaison between administrative and clinical staff and physicians. Assists and supports patient discharge planning as needed. Provides patient education regarding care and medications. Carries out patient care orders as directed by physicians per hospital policy. Assists with patient admissions and discharges as required. This position is responsible for coordinating all aspects of pre and post-transplant care. Participates in educational programs regarding transplantation for various groups. Experience with transplant populations is required.
