Job Title: Registered Nurse – Transplant Coordinator (Contract)

We are seeking an experienced Registered Nurse – Transplant Coordinator to support their Living Donor Transplant Program, with a primary focus on kidney and pancreas transplants. This role involves coordinating patient care across the full transplant journey, including evaluation, surgery, and post-transplant follow-up.
ScheduleShift: Day (5x8-hour shifts)
Hours: 8:00 AM – 4:30 PM
Schedule: Monday–Friday
On-Call: None
Weekends/Holidays: Not required
Coordinate care for transplant patients (pre, peri, and post-transplant)
Monitor patient progress and identify complications (infection, rejection, etc.)
Educate patients and families on medications, recovery, and lifestyle changes
Collaborate with multidisciplinary transplant teams
Manage transplant documentation and regulatory compliance (UNOS, CMS)
Track patient outcomes and maintain transplant databases
Assist with organ offers, allocation, and recipient preparation
Living donor kidney and pancreas transplants
Pre- and post-transplant patient care
Chronic conditions such as ESRD, CKD, and transplant graft failure
Work Environment
Acute care hospital and outpatient transplant clinic
Collaborative transplant team with interdisciplinary support
May float to other CPMC locations based on census
