Are you an experienced RN with strong care management and post-acute experience who thrives on coordinating care and improving patient outcomes?

We’re looking for a Care Management Coordinator to join our team and help ensure patients receive the right care, in the right setting, at the right time.

What You’ll Do

• Perform telephonic or onsite utilization management for inpatient admissions.
• Review medical necessity, continued stay, and length of stay using established criteria.
• Identify opportunities for alternative levels of care and timely discharge.
• Collaborate with physicians, hospital utilization review teams, case managers, patients, and families.
• Identify discharge planning needs early and facilitate appropriate transitions of care.
• Refer appropriate cases to Case Management and Disease Management.
• Present cases that do not meet criteria to Medical Directors for review.
• Identify quality-of-care concerns and delays and escalate appropriately.
• Maintain accurate documentation and ensure decisions comply with state, federal, and accreditation requirements.
• Build strong provider relationships while delivering exceptional customer service.
• Identify utilization trends and contribute recommendations for improvement.
• Support provider education related to managed care.

What You Bring

✅ Active PA RN License — Required
✅ post-acute experience — Required
✅ Minimum 3 years of acute care clinical experience in a hospital or healthcare setting
✅ Discharge planning and/or utilization management experience preferred
✅ Strong clinical judgment and communication skills
✅ Ability to collaborate effectively with physicians, providers, patients, families, and care teams
