Job Summary

We are seeking a highly skilled and compassionate Wound, Ostomy, and Continence Nurse (WOCN) to join our dedicated healthcare team. This specialist role involves acting as a key consultant for both patients and staff, providing expert guidance and direct care in the areas of skin integrity, wound management, ostomy care, and continence needs.

Based in State College, Pennsylvania, this position focuses on delivering comprehensive, evidence-based care. The WOCN will be instrumental in developing and implementing individualized care plans, educating patients and their families, and supporting fellow healthcare professionals to ensure optimal patient outcomes across various clinical settings.

Key Responsibilities

• Serve as an expert consultant to patients and staff regarding skin prevention, wound assessment and treatment, ostomy management, and continence care.
• Provide specialized assessment, planning, implementation, and evaluation of care for patients with complex wounds, ostomies, and continence issues.
• Develop and implement individualized care plans to promote healing, prevent complications, and enhance patient quality of life.
• Educate patients, families, and healthcare staff on best practices, self-care techniques, and ongoing management strategies for wound, ostomy, and continence conditions.
• Act as a liaison between patients, families, and the interdisciplinary healthcare team to ensure seamless communication and coordinated care.
• Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes.

Required Qualifications

• Completion of an accredited Registered Nurse (RN) degree program.
• Minimum of 3 years of full-time professional nursing experience, with a focus on medical-surgical patients.
• Board Certification in Wound, Ostomy, and Continence Nursing (WOCN).
