Responsibilities
  • Perform comprehensive wound assessments for acute, chronic, surgical, diabetic, pressure, and vascular wounds
  • Develop, implement, and evaluate individualized wound care treatment plans
  • Perform advanced wound care procedures, including dressing changes, negative pressure wound therapy (NPWT), debridement assistance, and ostomy care as appropriate
  • Monitor wound healing progress and adjust care plans based on patient response
  • Collaborate with physicians, surgeons, dietitians, physical therapists, case managers, and interdisciplinary healthcare teams to optimize patient outcomes
  • Educate patients, families, and caregivers on wound prevention, treatment, nutrition, and home care management
  • Document assessments, interventions, wound measurements, and patient progress accurately in the electronic medical record (EMR)
  • Assist with pressure injury prevention initiatives and hospital quality improvement programs
  • Ensure compliance with infection prevention standards, patient safety guidelines, and evidence-based wound care practices
  • Provide consultation and clinical support to nursing staff regarding wound management best practices
Requirements
  • Active California Registered Nurse (RN) License
  • Minimum 2 years of recent Wound Care nursing experience
  • Current BLS Certification required
  • Wound Care Certification (WCC, CWCN, CWON equivalent
  • Strong knowledge of wound assessment, treatment modalities, pressure injury prevention, and ostomy care
  • Experience with negative pressure wound therapy (wound vacs) preferred
  • Proficiency with electronic medical record (EMR) systems
  • Excellent assessment, communication, documentation, and patient education skills
  • Ability to work independently while collaborating effectively with multidisciplinary healthcare teams
  • Commitment to delivering evidence-based, patient-centered wound care
