Certifications: AHA BLS, Certification in Wound Care (various options available)

Description:
Conduct health assessment interviews focused on wound, ostomy, and continence related care.
Perform focused physical assessments and develop plans of care based on diagnoses and individual client needs.
Recommend medications and treatments as appropriate.
Develop, revise, and evaluate standards of care.
Assist in the selection of supplies, technology, and equipment.
Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wounds treatment.
Evaluate the quality and delivery of care provided.
Work collaboratively with the healthcare team and make referrals to other disciplines as indicated.
Teach, counsel, and assist individuals and families in assuming responsibility for prevention of complications related to ostomies, wounds, or continence problems.
Develop, provide, and evaluate staff education programs, including new employee orientation and continuing education programs.
Routinely conduct comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures.
Escalate and communicate challenges, including ethical issues and conflict resolution.
Collaborate with nursing leadership to initiate and recommend best practices based on assessments.
Participate in quality improvement activities.
Provide culturally competent education to patients, families, and staff.
Maintain the security and confidentiality of patient data and information.
Engage in activities to maintain a level of professional competency.
Identify available community resources helpful to clients and assist in utilizing them.
Manage asset related to rented equipment.
