Occupational Therapist - SNF Setting

Assignment Duration

This is a 13-week contract position.

Schedule

8 hours per day, 5 days a week (flexible).

Job Description

The Occupational Therapist will be responsible for the evaluation and treatment of both short-term and long-term patients within the Skilled Nursing Facility (SNF) setting. Under the supervision and assistance of Certified Occupational Therapy Assistants (COTAs), the OT will ensure timely documentation and provide effective treatments.

By developing individualized treatment plans based on physician orders, the OT will assist residents in improving their independence in activities of daily living (ADLs), functional mobility, cognition, upper extremity function, and overall quality of life. The OT will collaborate with an interdisciplinary team to promote a safe discharge and maximize each resident's functional potential through various treatment approaches.

Responsibilities

  • Evaluate patients and create tailored treatment plans.
  • Implement therapeutic interventions to enhance patients' independence.
  • Document patient progress and treatment outcomes in a timely manner.
  • Collaborate with an interdisciplinary team to coordinate care.
  • Supervise and support COTAs in delivering effective care.

Required Experience / Certifications / Licensure

  • Active Occupational Therapy license in Ohio.
  • Experience in a Skilled Nursing Facility (SNF) preferred.
