Home Health Occupational Therapist responsible for providing evaluation and re-visits in north-eastern Sacramento, Folsom, and Woodland - Yolo county service areas.

Responsibilities:

• Perform an average of four visits per day, including evaluations and revisits.
• Document patient care using the Cerner charting system.

Requirements:

• Two years of experience in a healthcare setting required.
• BLS Certification and NPI number required.
• Current California OT license required.
• Valid driver's license and proof of automobile insurance required for home health.
