Travel Occupational Therapist (OT) – Skilled Nursing Facility
Location: Santa Rosa, California
Contract: 13 Weeks
⏰ Hours: 36 Hours/Week
Shift: Varies
Setting: Skilled Nursing Facility (SNF)
We're seeking an experienced Occupational Therapist (OT) for a 13-week travel assignment in a Skilled Nursing Facility. The OT will provide therapeutic services focused on improving patients' functional abilities, independence, and quality of life.
✅ Requirements
Active California Occupational Therapist License
Minimum 3 years of Occupational Therapy experience
Skilled Nursing Facility (SNF) experience required
Ability to provide occupational therapy services to a diverse patient population
Strong assessment, treatment planning, and documentation skills
Clinical Experience
Evaluate patients' functional abilities and occupational needs
Develop individualized treatment plans based on patient goals
Provide therapeutic interventions to improve mobility, ADLs, and independence
Monitor patient progress and modify treatment plans as needed
Educate patients and caregivers on therapeutic techniques and safety
Collaborate with nursing staff, physicians, and other members of the interdisciplinary team
Maintain accurate and timely clinical documentation
Support safe transitions and continuity of patient care
