Required Qualifications: Valid California Occupational Therapist (OT) license; BLS certification; valid driver's license; auto insurance and registration; annual TB test; MMR, Varicella, Hepatitis B, Tdap (valid within 10 years), annual Flu vaccination; Drug screening; Physical examination
