Key Responsibilities

• Provide occupational therapy evaluations and treatments to home health patients in the Concord and Walnut Creek areas
• Actively manage your caseload to maintain expected productivity targets, including proactively backfilling cancelled visits
• Document patient care accurately and in a timely manner using Home Care Home Base (HCHB)
• Coordinate with the home office team to ensure continuity of care and optimal scheduling
• Track and report mileage in accordance with billing guidelines

Required Qualifications

• Active Occupational Therapist (OT) license in the state of California
• Minimum 1–2 years of occupational therapy experience required
• Home health experience required
• Proficiency with or ability to learn HCHB (Home Care Home Base) documentation software
• Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

• Prior experience with HCHB documentation platform
• Demonstrated ability to independently manage a home health caseload and maintain productivity
• Familiarity with the Concord/Walnut Creek geographic area
