Job Summary:
Occupational Therapist (OT) specializing in Home Health services in the North Napa Valley area. This position involves providing comprehensive home-based therapy to geriatric patients, with a focus on improving patient outcomes through timely and effective clinical interventions. The role requires flexibility, excellent communication, and adherence to documentation standards.
Job Details:
- Schedule: Monday through Friday, 8 hours per day, with a minimum of 40 hours per week.
- Required Qualifications: Valid license in hand, minimum of 1 year of relevant experience, OASIS experience required for staff completing Start of Care (SOC), proficiency in Homecare Homebase (HCHB) required.
- Preferred Qualifications: Experience with geriatric patient care and home health therapies.
- Certifications: BLS certification required.
- Experience: Minimum of 1 year of relevant occupational therapy experience; recent home health experience with at least 6 visits daily and a minimum of 30 points weekly for productivity preferred.
- Special Requirements: Active drivers license, personal transportation required, timely communication and documentation, scheduled orientation on Monday, and a flexible approach to territory coverage.
- Additional Information: Travel Pairs are accepted; each traveler must have own transportation and be assigned to their own territory. Modules are billable and completed during orientation. On-call rotation may be required and will be discussed during interview.
