Job Details:
• Minimum 1 year of experience required; first-time travelers accepted
• Patient population focuses on geriatrics
• 6 visits per day with an average daily census of 115
• Full-time productivity is approximately 25 units per week
• HCHB (Homecare Homebase) experience required until 10/1/2026; Epic experience required after 10/1/2026
• If clinician does not have HCHB experience, an online HCHB course must be completed prior to starting
• No weekend shifts required
• Floating within facility and scope of practice as needed
• Maximum of 7 days requested time off per contract
• Mileage reimbursed at IRS standard rate, tracked through HCHB
• Coverage area includes Washington, Johnson, Jefferson, and Emanuel counties with occasional support in Burke, Screven, Bulloch, and Jenkins counties
• Average mileage is 50-100 miles per day
• Holiday work expectations apply based on how many major holidays fall within assignment period
• Scrub color: Any color
• Required certifications: BLS
• Key skills include adaptive equipment assessment, cognitive assessment, OASIS, plan of care, home assessment, hip fracture and total hip replacement, knee injuries and total knee replacement, discharge planning, and wheelchair evaluation
• Interdisciplinary support includes physical therapy
